POSITION: Consumer Case Worker JOB LOCATION: Los Angeles / Inland Valley STATUS: Full Time or Part Time JOB DESCRIPTION As a Consumer Case Worker, you will assist individuals with disabilities to resolve their obstacles so that they may live better lives. You will evaluate what steps are necessary to improve their situation and do the legwork to make the change happen. JOB SUMMARY The Consumer Case Workers provide one-on-one assistance to individuals with disabilities in various aspects of life including (but not limited to): budget development, money management, housing access coordination, educational/academic support, household management, coordinating healthcare, managing appointments, problem solving, behavior management, accessing financial resources, improving interpersonal skills, assistance with personal health, life skills, self-advocacy, community mobility, resources awareness and facilitating circles of support and personal/social development. ESSENTIAL RESPONSIBILITIES & DUTIES • Maintain good working relations with professionals, staff, families and persons served. • Assist the consumers that you are serving to develop a sense of identity, self-worth and independence. • Keep individual file on each consumer up-to-date. Complete all assigned duties and paperwork accurately and on time. • Attend all necessary training’s and staff meetings within the time lines established. • Ensure all medical appointments are completed within established timeframes. • Maintain and turn in, on time, to the appropriate individual time sheets, progress notes and attendance reports. • Assist in the development of a variety of community activities and experiences providing opportunities for adults to assume and maintain valued roles and relationships. Show innovation and insight in defining roles; develop community's ability to provide supports. • Assist the person that you are serving to develop a sense of identity, self-worth and independence. Facilitate starting and maintaining friendships in the community. • Encourage and maintain active teamwork. Problem solve in a professional, non-hostile manner.
